There are some cases when we recommend women come to the office earlier (when they have a positive pregnancy test), such as if a woman has had recurrent early pregnancy losses or if she needs to be on blood thinners for a known blood clotting disorder. I live in CA and have Kaiser as my health care provider -- i was assumign that since they are an HMO and tryo to keep costs down, they wouldnt see me until 11 weeks or something! I had an ectopic pregnancy 2 years ago and they told me that it's kind of pointless to do an appt before 6-8 weeks because it won't really show up on the ultrasound before then. The reason they want higher risk people to come in sooner is because they can do blood and urine tests to make sure everything is ok and talk to them about possible signs/symptoms of trouble. If it's going to happen, it will, regardless of whether or not the OB has you come in. I am telling my parents before my first appointment, because i know they will be excited (especially because we recently had a loss in our family and we need some good news). The first one is with the nurse to take down complete health history of the mother and father and to do preliminary blood work. The next one is with the provider to do the physical/ultrasound and they wait that long to insure that they get a good view of the baby to estimate a due date. Patients can come in sooner if they have any issues such as history of early miscarriage, genetic problems or unsure dates. However, even after a good HSG showed my tubes and uterus were completely untouched by the fibroids...I spent all year fearing that I would be infertile (kind of irrational I know). Well, DH stopped "avoiding" pregnancy via charting and condoms in Aug 2009....and started actually trying in Dec 2009. I missed my period last Friday, skeptically took a test on Sunday, and the plus showed up RIGHT AWAY. I started hyperventilating, lol. Anyhow, I called a local OB (didn't realize my GYN isn't an OB anymore....thought he just quit doing deliveries) and they don't want to see me until MARCH 30!!! No confirmation blood test or nothing. I've taken 2 AHT since and keep telling the hubby "still pregnant"...but with my history of worrying over the last year I am going to go nuts before 3/30. Could I go to a walk in clinic or a normal dr to at least get a blood test? Sigh, God is working on me here...I know it. I am so excited and yet so scared to get excited! My OB won't even schedule an appointment until I've come in and taken a pregnancy test at the lab (which doesn't require an appointment) to make it "official". I've got my first visit at the end of next week. Fortunately, this week, I am on vacation, so that makes the time go faster. I think the timing of the initial visit has to do with the capacity of the ultrasound. Some of the first things my doc checks out is that the baby is in the uterus (i.e. not ectopic), whether there is one or more embryos present, and that the development is at about the right stage for the estimated timetable. Vaginal ultrasounds can confirm that between 6 and 8 weeks. Of course there was no heartbeat, but the Dr did see an "Embryonic Sac" So, things can been seen via U/S, but I don't think Dr's like to do them until a heartbeat can be heard as well. I am pregnant with my 2nd now, and went when I was about a month along. While they didn't do an U/S, they did check my thyroid levels - I take Thyroid meds - as well as Progesterone, HCG, and urine for UTI's etc. While you can't always prevent a MC, sometimes you can by making sure your Progesterone etc. levels are where they should be. After an ultrasound and sonogram, no one knows if I have an ectopic pregnancy or I'm too early on in my pregnancy to see the actual follicle in the uterus. This is probably why OB's ask patients to come in after 8 weeks so things are more clear and you avoid any uneccessary stress and worry. So now I have to rely on blood tests for HCG to tell me if I have a normal pregnancy or an ectopic. I went from feeling high one day to worried/stressed the next day. I have a question for all of you! --> Do they calculate the number of weeks along you are based on the date of conception, the first day of your last period, or simple two weeks before conception (regardless of when you actually had you period)? I live in Canada and with my second pregnancy my first appointment was at 20 weeks! He listenened to the heart beat and everything was normal. I was a little big for size so I was scheduled for an ultrasound 12 weeks later (32 weeks) to make sure my dates were correct!!! Found out I was having twins!! What a shock! Went to 40 weeks and was induced on my due date...both were born good sizes and vaginally. I live in a smaller midwestern city and was seen right away by one hospital the day I found out I had a positive. They are a new hospital just opening and did all the tests that day (I was 4 1/2 weeks). I went there because my regular doc and clinic was busy until the next week. I have an appointment with my regular doc at 6.5 weeks. That seemed long enough. This is my first pregnancy and I am really glad I could meet with a doc early to discuss everything. Due to high-risk with that pregnancy, I was scheduled asap with my daughter, and now, with my third, they've scheduled me next weds. but recommend that I see a high risk doctor this week if possible. I'm 6-7 weeks in. I took my homes prego test the day of my expected period and it was positive! my Dr. had me come in the next day. I found it to be very odd that Doc did a trans vag U/S. As expected, we saw nothing but my tilted uterus. He had me come in the very next week to do another trans vag U/S. He showed us a tiny black spec. It was my newly developed amniotic sac. He had us come in the next week to do yet another trans vag U/S. He was looking for a heart beat. He showed us faint shadow in my sac, that would eventually be an embryo. He couldn't locate the flickering heart beat. He turned to me and the daddy-to-be and said "I'M PESSIMISTIC ABOUT THIS, THERE IS NO HEART BEAT". He then told me to come in next week & if there is no heart beat, then we would have to consider this an abnormal pregnancy.Needless to say, I FREAKED. My" baby's-daddy" took me home, and did some research on the internet. We discovered that a lot of OBGYN's won't have you come in until your estimated to be at 8-10 weeks, to avoid freaking the mommies out by showing them uneventful U/S. There are so many factors and so difficult to pin point the exact day when your little embryo should be showing off its fluttering heart beat. It's easy for us expecting mommies to freak and stress out, especially if this is our first rodeo. I wish that my Dr. would have advised me to wait until at least the 8th week. Waiting just a little bit more would have prevented a lot of grief and disappointment. I wish all of you ladies luck, be patient and please don't stress out. After all this is a waiting game, so by all means, do yourselves a huge solid one, & wait it out if you have to and remember that staring at a blank U/S screen is very unsettling and if you wait just a tad longer, you will feel the biggest relief once you see your little blessing on that screen.
