Q&A: Fertility treatment basics?

We’ve been trying to conceive for a year, and nothing is happening. We’re determined to have a child, though. What fertility treatments are out there?

Re: We’ve been trying to conceive for a year, and nothing is happening. We’re determined to have a child, though. What fertility treatments are out there?

The Bump Expert

The good news: About two thirds of infertile couples have children after treatment. The bad news: Many insurance plans don’t cover infertility treatments. (Thirteen states, however, mandate at least some coverage, and more and more HMOs are covering part or all of it.)

There are a myriad of options, depending on the cause of the problem. For men, medication can often correct impotence and premature ejaculation, and surgery can reverse low sperm count. A procedure called electroejaculation is also available, which sounds scary but is actually just an electric stimulus that elicits ejaculation. (It's especially useful for men with spinal cord injuries who can’t otherwise ejaculate.) Another option is surgical sperm aspiration, in which sperm is harvested from the male reproductive tract. (Specifically, from the epididymis, vas deferens, or testicle – anatomy class all over again!)

For women, oral or injected fertility drugs such as Clomid, Repronex, and Gonal-F can correct ovulation disorders. Injections boost your odds of having multiples- twins or more! Some drugs stimulate the pituitary gland; others go right for the ovaries. Laparoscopic surgery can correct blockages or other problems in the fallopian tubes. Assisted hatching, which helps get the embryo implanted in the uterus, is another option. If you have endometriosis, blocked fallopian tubes or unexplained fertility, your best bet is probably in vitro fertilization (IVF). In IVF, doctors marry sperm from your husband and a mature egg from you in a lab, then deposit the fertilized egg into your uterus three to five days later.

There are complications associated with many of these treatments, so definitely do your research before signing up. In the meantime, improve your odds of conceiving by giving up smoking and drinking, getting enough zzz’s and exercise, and eating healthfully. Foods rich in folic acid (beans, leafy greens, OJ) are important for you and your husband because they increase sperm count and prevent birth defects. Don’t go overboard with the diet, though – being too skinny can throw off a menstrual cycle and make it more difficult to conceive. Good luck!

re: Q: Fertility Treatment Basics?

My husband and I have been trying to concieve for a year now, with no luck. I am 24 and he is 22, his sperm has been tested and is normal, and I am ovulating and having regular periods like clockwork. We are having sex at least 3 times a week, slowing down a bit right before and speeding up during ovulation usually. My gyno suggested clomid, but I thought clomid was to cause you to ovulate, which I am already doing...what do you suggest? What do you think could be wrong with me?

re: Q: Fertility Treatment Basics?

Are there tests that your doctor can run to see if you have any fertility problems?

Sweet3heart |

re: Q: Fertility Treatment Basics?

I am so glad that someone asked this question. I have been wondering about this myself, and my doctor doesn't answer straight out. Why? All the info I read is about not ovulating regularly. What other fertility problems allow ovulation but prevent conception? I have a cousin-in-law whose mucus destroys the sperm of her husband so they had to have IVF, but how common can this be? And blockages, well, wouldn't that prevent regular ovulation? Wondering, too, if anyone has this info.

re: Q: Fertility treatment basics?

my husband and I had been trying for about a year when we went to a fertility doctor. My husband had his semen tested and he was fine, and they ran some tests on me and couldn't figure out what the problem was. My period is like clockwork too so i thought that I was ovulating. A few months later they did an ultrasound to see if my follicles were growing to release an egg. We found out that although my period was normal, i was not ovulating. I had one cycle of a drug called Femara, similar to clomid, but my doc said there were fewer side effects, and my hubby and I concieved. I am now 11 weeks pregnant. Apparently it does not always happen that way on the first cycle of the drug, but i have heard really great success stories from people using Femara. Maybe you could ask your fertility doctor to do an ultrasound to see if you are ovulating. Good luck

re: Q: Fertility treatment basics?

My husband and I have been trying to conceive for several years now. We are both 30. We just found out his count is great but he has really low morphology and motility. I start all my testing next month. They will test my hormone levels on day 3 and do an ultrasound. Then after that on around day 10 of my cycle I will have the HCG test done. Then we just wait and see what the game plan is.

re: Q: Fertility treatment basics?

I have been having some problems with my cycle after I stop taking birth control. So I went to mo Obgyn and was put on Provera to regulate my periods. My Husband and I have been trying now for 6 months since and have recently started taking clomid for the first time. I am now hoping that it takes even knowing that it maynot happening the first time around. But what else can I do to help increase my chances.Hey good luck to all

re: Q: Fertility treatment basics?

Don't forget about acupuncture, either alone or in conjunction with Western medicine! My husband suffered from low morphology and motility and through acupuncture, Chinese herbs, and supplements his numbers nearly tripled. In addition, studies have shown that acupuncture increases the effectiveness of both IUI and IVF.

Thank you epete! As a licensed acupuncturist, I have treated both men & women TTC. Alone, & working with western medicine I have had great results. As always, it is your choice, and you must do what is right for you - there are other options out there.

Q&A: Fertility treatment basics?

My Hubby and I have been trying for just over 2 years with no success. I finally went to see a reproductive specialist and she said I looked fine and I was ovulating and that my blood test came back normal. Also my DH was tested and his juice was fine to. She took an ultra sound and said that I might be PG but she couldn't say for sure. I hope I really am. I'm due to start my period tomorrow and I'm not near as crampy as I usually am and my acne isn't as bad as it usually is at this time of the month. My breasts aren't tender and sore like they usually are either. This is all so weird. If I'm not PG my dr. is going to start me on Clomid to help me ovulate an extra egg. I really don't want to have multiplies though. Lord willing...I pray I'm really pregnant cuz I'm tired of being let down EVERY month!

Q&A: Fertility treatment basics?

I am on my first cycle of clomid now after almost 3 years of not using any sort of birth control or prevention, and a year and a half of actively trying. Both my husband and I are in our early twenties and healthy. My mom has PCOS, and though my tests came back normal, I have many symptoms of it, so i'm skeptical. So far, I haven't has a single bad side effect of the clomid. In fact, quite the opposite. I'm euphoric on it, I feel happy and bubbly and productive. I wake up smiling, its great! There's just a slight bit of nausea when I take it, but I take it right before bed so it doesnt matter. Good luck to you, cross your fingers for me!

Q&A: Fertility treatment basics?

My husband and I have been trying for almost two years now to conceive. I found out the reason why we weren’t successful was that I had endometriosis, I had surgery back in May and after 3 months of trying on our own with no meds, we finally decided to take Clomid we started taking them last month on the 3 day of my period. The only side affect I felt was hot flashes but other than that nothing really. I'm Optimistic we also had an IUI done and I wait to test tomorrow Morning! Crossing my fingers! I did however felt strong cramping yesterday and the day before but no spotting. Can I be Pregnant!!

Q&A: Fertility treatment basics?

I heard that you can try clomid. It's a fertility drug, only thing is it increases your chances of having twins by 10%. I have been trying to get pregnant and nothing is happening. So I am going to try the clomid option

Q&A: Fertility treatment basics?

I was diagnosed with PCOS about 9 years ago. I always knew it would be a challenge to get pregnant and after TTC on our own my Ob/Gyn referred me to a reproductive endocrinologist. I am blessed to have infertility coverage and went through 6 staggered clomid cycles (with a surgery in the middle) before I had any success. I am happy to say my husband and I conceived using 150mg of Clomid an HcG shot along with an IUI. I've found out through all my work ups and talking to other women at my doctor's office that there are a myriad of issues related to infertility. Lack of ovulation is a problem for many women but others have anatomical issues (fibroids, blocked tubes, endometriosis) or hormonal imbalances that don't allow them to keep a pregnancy. I know of some women who had to seek the help of a hematologist to address blood based imbalances which affect infertility (I don't know what they are specifically, but such problems do exist(. All in all, you have to be your own advocate and be patent. All the medical work ups take time as does finding the right treatment plan. If you are not already seeking the help of a specialist I would start ASAP so you can get all the preliminary tests out of the way. Best wishes!

Q&A: Fertility treatment basics?

I have to advocate for acupuncture as well. I am 29 and we spent two years doing fertility treatments. About 6 months ago I started doing acupuncture in conjunction with fertility treatments. We noticed things happening that had not previously and I first stopped having to take a drug to induce a period and then I got to stop the shots to force me to ovulate. Then over the holidays because I was out of town so much and not able to make any of our appointments we took the time off from fertility treatments but kept up the acupuncture. After a month with no drugs and no treatments just acupuncture I got pregnant on my own. I know it would not have happened without the acupuncture. I would highly recommend it. Plus you feel take away a very relaxed feeling after each treatment. It's almost as good as getting a massage each week!

kathryntart |

Q&A: Fertility treatment basics?

How often did you get the acupuncture treatments??

jalthardt |

Q&A: Fertility treatment basics?

When I started acupuncture treatments she wanted me to do it 3 times per week for the first 3 months. It was getting so expensive...but yes, it is relaxing when I'm not freezing my butt off in the treatment room! LOL!

Q&A: Fertility treatment basics?

So I just noticed one of my older posts on here and wanted to update! Sadly, I wasn't pregnant and still haven't gotten pregnant. I didn't go back to my dr. for a few months but started seeing her again this month. I had an HSG done and my tubes were clear but my uterus was misshaped. She said that should not be a problem though. Now she is definately thinking that I have endometriosis. She gave me the option of doing laproscopic surgery or trying femara/letrozole fertility drug. I opted to try the drug to see what could happen. A woman who posted above said that she got pregnant the first time using femara. This is my 4th day taking it and I really hope I get the result we want since we have been trying on and off for over 2 and a half years. Good luck to every one else!

Q&A: Fertility treatment basics?

My husband and I had been trying for almost a year with no luck. After we saw a fertility specialist, I was diagnosed with PCOS, and my husband had a low count with low motility and morphology. So my husband went on vitamins to help his count, and I took 50 mg of Clomid with a shot (I can't remember the name!) to help my ovulation. We also had an IUI done. I am happy to say I am 5 weeks pregnant!! :) It only took one round of Clomid and one IUI. The only side effect I had from the Clomid was mood swings - I had them BAD haha!! But it is completely worth it!! :)

Q&A: Fertility treatment basics?

First I need to say - thank you so much! - As I am reading the posts my stress levels are going down just a bit. I am glad I am not in this Fertility Battle alone. Much appreciated. Now I have other questions to ask my fertility doctor:-) Ok so here it is.. I have gone to see my GYN and Fertility Dr. and have had all sorts of tests. I have done this three times! My DH kept wanting me to get checked out to make sure all was okay and that we can start TTC #2 - Well there is no need to go into the details - All my tests came back ok so far BUT.. my DH hasn't gone in for his testing:-( How do I get him to go? He has to get new lab work and papers again since he waited so long (or shoudl I say since he put it off)... I don't get it.. He loves me. I love him.. isn't that enough to go and get the tests done and to begin this journey Did anyone do anything special to get your DH to go? He made me go (and I really wanted to know if all was ok anyway) So then I am ok.. And .. What's the big deal - I just want to know if everything is ok then we can move on. I thought guys were logical.. it doesn't make him less of a man to me. It is just another situation to tackle (another obstacle to climp). You can't learn anything if it's smooth sailing all the time.. This could have been handled and over with a year ago!!! What should I do? Suggestions?

Tweetytaps |

Q&A: Fertility treatment basics?

Tweetytaps - When my husband and I were going in for testing, my doc allowed me to bring home the SA kit, which is where we got the sample and bring it into the lab without every having my DH step into the doctor's office. As long as the sample is turned into the lab w/in an hour (less than 40 mins is the optimal time frame) then they can read the sample. My DH was all for it especially since he had only to commit a few minutes of his time and no doctor visits. Maybe you can see if your doctor will work something out like this for you besides like you said.

Q&A: Fertility treatment basics?

My husband I were trying for a little over 2 years and nothing. I suffered from an eating disorder, which made my period go away, and it did not return even though my weight was back to a stable healthy weight! We thought that we'd never have kids... but our fertility clinic gave us the option of IUI. My husband and I both tested fine, and my HCG test was normal as well, so we thought we would give it a try. I was on clomid and a hormone needle for 5 days and drove to our clinic everyday so they could monitor my follicles and when I would surge. We were told that we would have a 15% chance of conceiving doing it this way and that it would probably take a few times. IUI is just when they take the sperm and pick the "good ones" and wash them and inser them directly into the uterus at the time when your eggs are dropping. The 14 days after were the longest ever, but it was ll worth it, and we are now in our 6th week. SO I would suggest researching all your options and even though they might not give you great rates of it working, it's all worth it in the end!

Q&A: Fertility treatment basics?

sweet heart I hope you got an answer before now, but plan b is only effective up to a few days after unprotected sex. If you hadn't used it by the 21 of august then it wouldn't stop a pregnancy. on the possitive side if your cycles are regular 28 day cycles you are probably not pregnant.

Q&A: Fertility treatment basics?

Ok! So I'm new to this site. I saw this question and it just fits! My husband and I have been TTC for about two years. We finally gave in and decided that we needed to go to the doctor to see what our next option was. My husband had to give a sperm sample and they had told us that he had a great count of 24 million anything over 20 million is good! But he had slow moving sperm. . . so we had to take other blood tests to find out if everything else was ok! And we both are fine besides his soldiers not wanting to march! On christmas eve this year we went in and we had IUI done, which is also known as artificial insemenation, where they take the sperm from my husband, clean it off and with a tiny catheter place the living sperm inside the uterus around the time you ovulate. They trigger ovulation with a shot called ovidrel. SOOO. . . . I had 2 eggs that were ready to be fertilized that day. They injected 2.4 million living sperm to the top of the uterus. January 7th is the day I go get blood drawm to see if we are pregnant. :) I guess my story just leads to the fact that this is the first thing the doctor said he would try with us since the only thing that is wrong was the swimmers not swimming. I hope this helps!

Q&A: Fertility treatment basics?

This all sounds exactly like what my huibby and I are going through. He has 2 kids by his first wife so we assumed it was all me. We did US and an HSG, all my results came back normal as well as all labs. We found out he had great sperm count but not a lot of mobility. I am on my first round of Clomid and should be ovulating today. I believe I am, one of the side effects is cramping during ovulation and it definately feels that way. We have been TTC for almost 2 years with no success. If we do not conceive within 4 treaments of clomid we were instructed to go see a Fertility Specialist for an IUI. FUN!

Q&A: Fertility treatment basics?

My hubs and I have been TTC for 10 months. My period has been incredibly irregular after getting off birth control 11 months ago. The first three months, I had nothing and now I get my period every 45 days...this time it took 70 days to have a substantial period. My obgyn says she cannot do anything until we have tried a FULL YEAR, but I've been reading much different stories on this page. Do you really have to wait a full year to get some help?? I had blood tests and a preconception checkup before we started trying and supposedly I was completely healthy and ready to get pregnant, so it's killing me that nothing is working now! Do I have to have consent from my obgyn to go to a fertility specialist?? Where can my husband get tested to see if his swimmers are any good?? Someone please help!!!

Q&A: Fertility treatment basics?

My hubby and I have been TTC since 05/2010. I have had one child, but that was almost 11 years ago now and then I went back on BC. My AF has been regular every month since stopping BC in 05/2010. We have been tracking my periods and having sex every other day but there has been no luck. So we when to my OB/GYN and she recommended getting my husband checked since he hasn't had any children. We got the results back and he has low mobility, but his count was fine. My doctor recommended IUI due to his low mobility. I am really nervous because we are supposed to start IUI this month. I just want this so bad that I am afraid that it wont happen because I want it so much!!

Q&A: Fertility treatment basics?

I am 30 and my husband is 34 and we have been TTC for 2 years. Recently we visited a fertility clinic to see what our options were. After taking the at home ovulation predictor kit to determine if I was indeed ovulating naturally, we coupled that with Chlomid to attract more folicles. It doesn't MAKE you ovulate. I did find that the electronic ovulation tests that may cost a little more, worked better as it read the silly lines for you... I got an empty circle when I wasn't ovulating and a big fat smiley face when I was! It may be more expensive... But WELL worth it! Try testing at least 7 days before you THINK you're going to ovulate so you don't miss it by accident (like I did). Because we just started Chlomid, we wanted to try and conceive naturally for a couple of months. We never got pregnant that way. We decided to try IUI (intra-uternine-insemination) This is used for people that are ovulating but not getting pregnant naturally.. So, after another positive ovulation result I called the Dr and scheduled an ultrasound to see how many folicles (eggs) I had. Your partner goes in and gives his sample, they clean it - take out all the slow ones, dead ones, etc. and then about an hour later you come into the office and they take a very long plastic syringe and literally put it through your cervix in hopes of catching that egg at just the right time! You wait a few days and hopefully it implants. Luckily, we only had to do this procedure once and I became pregnant! They said because I have a tilted pelvis, the sperm may be getting lost and can't find where to go (for lack of better technical terms). I am now 5 weeks pregnant and hoping for healthy pregnancy! It was a wonderful relief to know I was able to get pregnant! Good luck!

Q&A: Fertility treatment basics?

...and sorry, hit send too early. My main point that I wanted to say is switch docs to someone who listens to you better: I went to mine, despite only being off the pill for only 6 mo. and TTC for only 3, and he ran blood work immediately to start testing thyroid, hormones, etc. to eliminate possibilities. We'll evaluate options as soon as test results get back. He's thinking provera followed by Clomid as a possibility but waiting for results of course. While I know many doctors want you to try for a full year, that "rule" is for people who have regular periods; mine told me if I waited a full year that's just a waste of 6 more months. I clearly have ovulation issues, 6 more months is not going to change that. I think it is important to have a doctor who supports you, so you may want to consider switching. Good luck!

Naluanela |

Q&A: Fertility treatment basics?

I agree having a doctor who understands you and wants to help is so key. I have been off the pill for 7 months now and having been actively ttc for about 3, however I started noticing irregularities in me period, and was worried that it was affecting my ovulation. The doctor put me on a maverol to regulate my ovulation and we start clomid at the end of this cycle...so hoping that it works the first time! Good luck to everyone!

Q&A: Fertility treatment basics?

Once again I've miscarried and this makes 3 for me. We were all set to start IVF, the docs thought it would really help, especially since all our testing came back negative. Stupid NJ Mandate on health insurance won't let us however even thought the doctor told the insurance company it was a medical neccessity. Now we're being forced to have IUI which the doctor isn't hopeful about, but we need to have it fail 3 times before we can get IVF. :-\

Q&A: Fertility treatment basics?

My husband and I have been TTC for 2 1/2 years. We found out 6 months ago that he has a low sperm mobility but a high sperm count. Meaning he only has 16% that will move anywhere. So the Doctor says that we will have to do an IUI. This is where they collect and wash the sperm (separate the good from bad) then inject them into the uterus to the egg. I am also taking Clomid just to regulate my ovulation. The first time we did it 2 months ago it hurt because my cervices was to high and he had to pull it down to insert the tube. But the second time went in just fine. We are now waiting to see if the second round worked. Only 7 more days to go.

Q&A: Fertility treatment basics?

My husband & I have been TTC to several years with no luck; I am 41 and he is 43. Since we were not sure what to do next, we went to an IVF informational seminar to see what it was all about and got pretty freaked out with the whole process (I am not fond of needles and had no idea what was involved...yikes!) . I am not sure that we need all that (IVF) and started to ask some questions after the seminar. I met this amazing specialist who handles ALL stages of Fertility, not just IVF. So we set up an appointment and went for testing for both of us (my husband's sperm) and HSG for me (to see if any blockage with the fallopians). Turns out my husband's count was low, but quality was good. So I went through the HSG (painful) procedure but learned that I had polyps, fibroids and endometriosis. Quite scary to hear that but at least we had answers; since that was why we were not having any luck. So the next step was surgery (had to go through laproscopy & hysteroscopy at the same time) to clear out my uterus and remove the fibroids and polyps. I was quite freaked out at this point but knew that i needed to undergo surgery for my health, as well as prepare my body for conceiving. Surgery was a success and I felt fine after wards. We wanted to try to conceive naturally for a few months to take a break from the testing, surgery, and stress of it all. It's now the 3rd month after surgery with still no luck, so our Dr prescribed 100mg of Clomid for 5 days (today is day 4; with no severe side effects..yay). We are now scheduled for IUI (Insemination) next week after Clomid takes affect and praying this is the solution to get the low count but high quality sperm to where it needs to be. Age 41 and not getting any younger. OH BTW, (ironically) my assistant, my best friend's daughter and a college friend (my age) are ALL Pregnant around me....So...let's hope it's now MY TURN....prayer please ;-)

My husband and I have been TTC for three years. I had brain surgery in 2008 to remove a prolactinoma that had caused me to stop having periods all together. Well now, my period had come back pretty regular (not like clockwork but considering i hadn't had one in four years- they definately were back lol). Now I've been heavily spotting for two months, esp after sex. Some of my girlfriends think i may be pg, and some think something could go wrong. We havent had insurance so I have put of going to see a doctor, but I have an appt tomorrow and now I'm terrified! Any help ladies?

Q&A: Fertility treatment basics?

My husband & I are on our 6th miscarriage. 4 of the 6 we have lost due to the baby losing its heartbeat between weeks 7 & 8. I've been to a infertility specialist for the 4 and 5th pregnancy...and the specialist was in shock when there was no heartbeat. He said they both grew to between 7 & 8 weeks..but lost they heartbeat. Anyone else have this problem? Specialist flat out told my husband & I that he didn't have a clue why this was happening. I've been through MANY different tests, chromosomal testing on fetus came back normal. Can anyone relate??? This has been going on for 4 years now.

A "natural alternative" I recommend is Mayan massage. Find a certified massage therapist to teach you the correct technique an use it every day. Bonus: helps with the digestive system too.

Q&A: Fertility treatment basics?

Just found this article on sciencedaily.com about how "insufficient progesterone is one reason for infertility in some women," which are "Immune Cells Essential to Establishing Pregnancy." "Environmental factors such as infection, obesity and stress all contribute to inflammatory responses and affect the generation and function of macrophages in women. This could therefore impact on the macrophages' ability to support pregnancy," http://www.sciencedaily.com/releases/2013/07/130708142949.htm

Q&A: Fertility treatment basics?

My husband and I have been TTC for a year and a half and not having any luck. We went through all the testing and luckily his testing came back normal and my tubes aren't block, BUT I'm not ovulating. I just finished my first round of Clomid and I'm praying it works. I am experiencing one side effect in particular with it... mood swings. Not fun! However, I know it will be worth it in the end.

Q&A: Fertility treatment basics?

You have plenty of options. My husband and I have been trying for 5 years and we just started out IVF process yesterday. We did IUI's with clomid 5 times. They found I have endometreosis. I had surgery to correct it and we are now starting the IVF shots. You first step is talking to your OBGYN. If you have already done that, you need to see a fertility specialist.

Q&A: Fertility treatment basics?

My husband & I have been trying for almost 3 years, we didn't want to push the fertility issue until we got married. He just went to have a seamen analysis done after Christmas, it came back normal. All of my tests came back normal & my OB plans on starting me on Clomid tomorrow. Fingers crossed.

Q&A: Fertility treatment basics?

We began TTC in August 2013 and got pregnant in September. Unfortunately I had a chemical pregnancy (miscarriage before implantation) at 4 weeks. We took October off from TTC and tried again in November; again another chemical pregnancy at 4 weeks. My OBGYN has told us to take two cycles off and to come in for hormone testing in February. Since the fertilized egg is shedding before implantation, there's a chance my progesterone levels are low or off balance and affecting the uterine lining. Sometimes doctors will not move forward with testing until after 4-5 chemical pregnancies so I'm lucky he is willing to test next month. It has been a frustrating roller coaster but thank you all for providing your stories to help me get through this difficult time. Hopefully the tests will come back with some helpful results and we'll be able to move forward. Good luck all and never give up HOPE!

We've been trying to conceive since last July. In March we made an appointment to see a fertility specialist. Hi sperm is fine and I ovulate normally. Last month the doc put me on femara (letrozole) and it increased my eggs from 1 to 3...unfortunately I still got a BFN. We are doing the same regime this month...hopefully with different results. Ill keep you posted

Q&A: Fertility treatment basics?

My husband and I have been TTC for over 3 years. He has varicoceles - got the surgery to fix it but he is one of very few where the surgery made it worse. His sperm count is low and the motility and deformity is bad. We tried one IUI with only 5 million sperm and that didn't work. Went back for a second and there was less than 2.5 million so we didn't go through with the procedure. We took some time off, researched, gave it another go naturally, and now will be starting our IVF cycle this week. Praying for a healthy pregnancy as this is our last option simply due to finances.
